If you are submitting your 12 month I-983 evaluation, please submit this form and attach the I-983.

STEM OPT 12 month Report

If you are reporting your 18 month STEM Extension employment, please submit this form (no I-983 is necessary).

STEM OPT 18 month Report

If you are submitting your 24 month I-983 evaluation, or submitting final report for an ending employment, please submit this form and attach the I-983.

The 24 month / Final report must be submitted no later than 10 days from the ending of employment.
