- STEM OPT Extension Request
To request 24 month OPT STEM extension recommended I-20, submit below form with completed I-983 (page1-4)
- STEM OPT Change of Employment Report
If you have changed your OPT STEM Employment, please submit this form and attach the I-983.
- STEM OPT 6 month Report
If you are reporting your 6 month STEM Extension employment, please submit this form (no I-983 is necessary).
- STEM OPT 12 month Report
If you are submitting your 12 month I-983 evaluation, please submit this form and attach the I-983.
- STEM OPT 18 month Report
If you are reporting your 18 month STEM Extension employment, please submit this form (no I-983 is necessary).
- STEM OPT Final / 24 month Report
If you are submitting your 24 month I-983 evaluation, or submitting final report for an ending employment, please submit this form and attach the I-983.
The 24 month / Final report must be submitted no later than 10 days from the ending of employment.
